Northern Territory Pathways to Community Control program

Action description: 

The Northern Territory (NT) Pathways to Community Control (P2CC) program transitions Aboriginal primary health care services from NT Health to Aboriginal community control. Transition to community control, also referred to as regionalisation, is a complex task and long term agenda. The regionalisation agenda aims to increase the involvement of Aboriginal communities in health decision making and improve service delivery and outcomes through better coordination and integration of services.
