1.2 The Conflict of Interest policy/procedure/guideline (or equivalent) that applies to staffing decisions (including recruitment/selection panels, delegates/decision-makers), including any guidance about declaring family or personal relationships.

1.3 Any standard template or form used for conflict of interest declarations in recruitment/selection processes (e.g., panel member COI declaration, delegate COI declaration), including where the COI declaration is a section within another standard template.

1.4 Any standard selection report / panel report template (or equivalent) used for recruitment/selection that includes a conflict of interest declaration/record.

1.5 Any standard template used to justify/approve non-advertised engagements / direct engagements / exceptions to advertising (however described) where it includes a conflict-of-interest declaration or conflict check.

1.6 Any standard template used to approve acting arrangements / higher duties / temporary transfers (however described) where it includes a conflict-of-interest declaration or conflict check. 

1. A. Systemic audits/reviews (final only):

   1.1 Any final internal audit reports, assurance reviews, governance reviews or external consultancy review reports held by NIAA that assess recruitment/selection, promotion or acting arrangements and which specifically address (in whole or in part) nepotism/favouritism, conflicts of interest or discrimination.

   1.2 This includes any final management action plan or final implementation/status report that accompanied such a report.

2. B. Aggregate governance reporting (final only)

   2.1 Any final dashboards/briefs/slide packs/reports provided to NIAA senior governance forums (for example: Executive Committee, Audit and Risk Committee, People/Workforce committee or equivalent) that summarise, in aggregate/de-identified form, either:

       2.1.1 numbers of recruitment/acting/promotion-related conflict of interest declarations or identified breaches/issues; and/or

       2.1.2 numbers of complaints/allegations and outcomes relating to nepotism/favouritism, conflicts of interest in staffing decisions, or 

       2.1.3 discrimination/bullying/harassment/victimisation in the workplace.

4. Any briefing or advice from NIAA to the Minister or her Office related to either the approval of ALC Budget cover or the approval of funding for termination payments relating to Mr Hewitt and/ or his spouse.

5. Any documents signed by the Minister or her delegate related to the financial and other arrangements associated with the termination of employment of Mr Hewitt and his spouse from employment with the ALC, associated Aboriginal corporations and/or Winchelsea Mining Corporation."
